To improve follow-up of screening results, a simplification of organisation and guidelines, introduction of quality assurance systems, and better coordination between authorities and between different levels of health care are all required.
Official guideline introduction of adjuvant radiotherapy for locally advanced breast cancer (T3/M0 or any T,N2-3/M0), was in 2002 and a large variation existed before and afterwards (Table 3).

Although before-after observational study was the best design, we recognise that it is not of as high methodological rigour as a randomised controlled trial and does not control for secular changes that may have occurred in the absence of guideline introduction.
The United States Environmental Protection Agency EPAA) has issued guidelines for introduction of genetically engineered (GE) species that state that GE organisms must: (a) Be contained to specific environments of introduction, (b) Not out-compete native species for resources, (c) Minimize foreign gene contamination (horizontal gene transfer) into native gene pools [ 8].
CFR system introduction guidelines were developed using the results of individual surveys, trials, and other countries' guidelines [6 9].
Finally, we developed introduction guidelines and introduced a CFR system into one selected remote area to test our guideline's efficacy in introducing a successful CFR system to our country.
We were able to discuss and develop CFR introduction guidelines for Japanese communities using the results of the individual surveys, pilot trials, and other countries' guidelines.
