One investigator (VR) prepared tables and other graphical indices to identify patterns and themes, whereas the interview guide categories served as a preliminary code-developing framework.
We used the WHO technical guidelines classification (WHO 2002; Table 2), rather than the simplified prevalence based field guide categories that are now used to determine treatment frequency (WHO 2006b; Table 2).
