Typically the nursing process consists of the five phases: general assessment, nursing problem/diagnosis, goal, intervention planning and documentation and evaluation [ 28].
In the FIN-D2D program, only 29% of the participants had three or more sessions in usual primary healthcare [ 14], and 43% and 57% of the participants attended a maximum of six counselling sessions in the GGT Diabetes Prevention Project and the GOAL intervention, respectively [ 11, 12].
To determine whether the results obtained in clinical trials could be replicated in "real world" primary health care settings with limited resources and existing personnel, the GOAL intervention study [ 11], a lifestyle implementation trial using a structured group programme, was designed in Finland.
For example, maternal mortality was an important element of the MDGs, but under a health goal interventions focused on service delivery (pdf) rather than on the deeper causes of gender inequality – unsafe abortion, early marriage, violence against women, male dominance in household decisions about spending on health, and so on.

This work was carried out on the premise that disability is a multi-dimensional phenomenon involving not only the child but also its environment, and that establishing eligibility includes implicitly or explicitly postulating goals, interventions and needs.
