At the global level, maternal mortality has decreased at an average of less than 1% annually between 1990 and 2005, far below the 5.5% annual decline necessary to reach the 5th Millennium Development Goal (MDG), that is to reduce maternal mortality by three-quarters by 2015 [1].
This study was based entirely on data for England, but we believe that cause specific analyses could provide much greater insight into socioeconomic inequalities in neonatal mortality on a global level and allow all countries to more fully understand their early childhood mortality rates and prioritise appropriate interventions.
As the above-mentioned systematic review highlighted the need for knowledge about the macrostructural causes of maternal mortality [ 7], the present study investigated the relationship between some macrostructural factors and maternal mortality at the global level in 2010.

The need to improve maternal mortality has been recognized at a global level by including it in the Millennium Development Goals [ 6].
Under-5 child mortality estimates are produced at the global level by the Inter-agency Group for Child Mortality Estimation, which includes UNICEF, WHO, The World Bank, the UN Population Division, Harvard University and others.
