Initially, the program was based in written formulary requests and telephone/fax contacts to provide authorizations for tests to be performed.
Additionally, at least in the Veterans Administration health system, bevacizumab was available prior to formulary listing but after FDA approval via non-formulary request (personal communication, Geraci M, 2010).
