Data were analysed in Mplus 6.1, using robust (mean- and variance-adjusted) maximum likelihood estimation (MLMV) for continuous indicator variables.
Second, the number of correct responses in each phase were added for each participant to produce four continuous indicators, namely, number of questions asked on present illness (range 0 10), number of signs sought in clinical examination (range 0 6), number of times lifestyle advice was given (range 0 3) and number of other necessary advice (range 0 2).
