The old form, which required a handwritten explanation and thus discouraged some busy officers from completing it, is being replaced by a simpler one that provides a checklist of reasons for the encounter, like "furtive movements" or "fits description".
If you do not list anybody, please type either "No one fits description" OR "I prefer not to answer".

NYPD Officer Edgar Gonzalez in Brooklyn's 88th precinct checked "Fits Description" as a basis for most of his 134 stops during a 3-month period in 2009.
