Patients (N = 61) were recruited from 2 university-affiliated, outpatient, heart failure programs.
Trials with a follow-up period of <3 months, with inadequate randomisation techniques, or those reporting on heart failure programs, were excluded.
Heart failure care models inspired by the chronic care model, also known as heart failure programs or heart failure units, have shown clinical benefits in high-risk patients.
"They don't want to bother the doctor," said Maureen Carroll, a heart failure program coordinator at the center.
"I don't think there was any other prospect for keeping him alive," said Dr. David Rosenthal, director of the pediatric heart failure program at Lucile Packard Children's Hospital, at Stanford University.
With a double engine failure programmed into the simulator, he said, the plane veered to one side while rolling to the other.
"It's been striking to me since I went into practice how unhappy patients are and, frankly, how mistreated patients are," said Dr. Sandeep Jauhar, director of the heart failure program at Long Island Jewish Medical Center and an occasional contributor to Science Times.
A nurse known as a heart failure program coordinator spends at least one hour explaining the patient's condition to the patient and his or her family, and preparing them to manage it outside the hospital setting.
As patient volumes increased, the program grew accordingly and has developed into an integrated heart failure program, which is part of a larger multidisciplinary team.
