Mike Farrar, former chief executive of the NHS Confederation, came to speak to Health Foundation staff recently and pointed out that we have seen "responsibility [for safe care] increasingly externalised" to regulators and national bodies.
The understandability of the reasoning process needs to be considered at the time of designing a negotiated learner model in terms of the complexity required and the extent of the reasoning process that can be usefully externalised to the learner.
Despite this, current environmental decision-making strategies often fail to incorporate these non-linearities because impact assessments are regularly performed independently without considering contributions to future cumulative impacts, which are, instead, externalised to the environment.
