11 A comprehensive literature review using the databases MEDLINE, EMBASE, CINAHL, British Nursing Index and Cochrane Central Register of Controlled Trials (CENTRAL) will be performed; an example of search strategy in MEDLINE is shown in online supplementary file 1.
