More examples of questionnaire used for evaluation of physical design can be found in automotive industry.
Notable examples of questionnaire re-engineering are the Impact of Visual Impairment (IVI) for low vision outcomes [ 33, 34], the Visual Disability Assessment (VDA) reinvented as the Cataract Outcomes Questionnaire [ 28, 35], and The Visual Functioning 14 (VF-14) [ 36- 38].
Examples of questionnaire webpages are shown in Figures 1 and 2. The questionnaire functionality was also extended in an imaging module to allow the quantitative coding of MRI findings, including allowing for multiple simultaneous research projects (approximately 2,230 MRI variables collectively across all coding models).
Examples of questionnaires which can be administered to both healthy and chronically ill children are the TAPQOL [ 4], TACQOL [ 5], Kidscreen [ 6] and CHQ [ 7].

There does, however, appear to be discrepancies between the patients' and clinicians' perception of the problem, for example, a number of questionnaire studies [ 17– 22] would appear to suggest that DHS was not considered to be a major problem by patients and, as a consequence, do not normally self-treat or seek treatment from their dentists.
