The results from the enrolment and utilization pattern suggest that just as the enrolment coverage has been increasing over the years since the inception of the NHIS, so has the total count of health facility visits as well as the number of individuals making those visits.
Selected process-related data variables were entered at the field level to enable rapid feedback on essential implementation variables (e.g. enrolment, coverage and timing of home visits, etc).
Most of these SHI schemes are however facing challenges such as low enrolment coverages, poor quality of healthcare services, inefficient provider payment mechanisms, high claims bill, moral hazard and adverse selection which have the potential of eroding the gains made [2, 3].
However, health systems issues such as poor quality of services and lack of patient empowerment remain barriers to increased treatment-seeking [ 10, 11], while low enrolment limits coverage of CBHIs and similar voluntary schemes [ 12].
Firstly, key indicators from MICS 2000 and SMS 1999, including infant mortality, child mortality, maternal mortality ratio, literacy, female literacy, primary school enrolment, Immunisation coverage, malnutrition rate, access to safe drinking water, clean safe delivery, HIV/AIDS knowledge, etc., were used to rank and select nine "focus states" as the most vulnerable in northern Sudan (Figure 3).
With the bulk of states, and the US population, coming under the federally-run exchanges, the onus is now on Washington to set up its scheme by October 2013 for the start of individual enrolments, with coverage through the exchanges starting in January 2014 – a massive task that could determine the ultimate success or failure of Obamacare.
