To begin with, the designers of Word apparently believe that the conventional method of endnote numbering is with lowercase Roman numerals — i, ii, iii, etc.
You quickly discover that, by continually hand-adjusting the typewriter's platen (the "roller," in layman's language), in order to superscript your endnote numbers, you have thrown the alignment out of whack, and when you roll the page back up to underline the ibid.s you type the line right through the word.
The numbers returned were then matched to EndNote record numbers.
By default, endnotes are numbered with Roman numerals (i, ii, iii, etc).

The de-identified data are available on request, but in reporting the findings, we included superscripts linked to a combination of letters and numbers (see Endnotes) referring to communities or PHC managers when the context, mechanism or outcome being reported occurred, was identified or described in that community or by the PHC manager.
If using footnotes or endnotes, make sure that the citation matches the notation number used in-text.
