Records were sorted to eliminate articles (UK or US) when the corresponding author's address listed did not match the address of that individual when the research was conducted.
Predetermined eligibility criteria were used to eliminate articles beyond the scope of this review.
The resulting articles were first screened by title, then by abstract, and finally by full text to progressively eliminate articles not meeting the inclusion criteria.
It should be noted that the quality assessment tool did not aim to apply a specific cut-off point to eliminate articles of seemingly poor quality.
The eliminated articles comprised duplicates, case reports, letters to editors, commentaries and studies with no quantitative wait time to surgery data (Table I, Fig. 1).
An additional 37 articles were eliminated after reviewing the abstract; we eliminated articles by abstract only if they were case reports, erratum, reviews, or small case series (less than 10 study subjects).

A similar proposal in the House would also eliminate Article II's "natural born" requirement, which Mr. Hatch said "was driven largely by the concern that a European monarch, like King George III's second son, the Duke of York, might be imported to rule the United States".
