Objective: To compare diagnostic and prognostic performances of conventional and novel cardiovascular biomarkers in 2 age groups: young (<75 years old) vs. old (≥75 years old) dyspnoeic patients.
Several biomarkers are used to diagnose acute heart failure (AHF) and assess prognosis of dyspnoeic patients, but their value in elderly patients is unclear.
After decompression, the patient remained disoriented, but was not dyspnoeic anymore and showed normal pupillary reactions.
Very often, this difficulty precludes the use of these measurements in children, the elderly and the very dyspnoeic patient.

AIMS: Experience with amino-terminal pro-brain natriuretic peptide (NT-proBNP) testing for evaluation of dyspnoeic patients with suspected acute heart failure (HF) is limited to single-centre studies.
On admission the patient was dyspnoeic, cyanotic, haemodynamically unstable, hypotensive (80/50 mmHg) and tachycardic (136 bpm), with low oxygen saturation (82%) in oxygen with Venturi mask (6 l/min), respiratory rate 30 breaths/min, and with primary hypoxemia and metabolic acidosis (pH 7.30, pO2 54 mmHg, pCO2 23 mmHg, HCO3 16.2 mmol/l).
