To explore possible effect modification by tobacco smoking and alcohol drinking, analyses were further stratified by these two habits, using the following categories: never, former, and current smokers; drinkers of <5 g per day of ethanol, 5 to <25, and ⩾25 g per day for women; and <10, 10 to <50, and ⩾50 g per day for men.
