In former days it was often difficult to distinguish between one article and the next, because distinctive headings or inset titles or the use of boldface was rare.
Four lists of distinctive headings, one for each PICO element, were manually created.

One solution is to use the structural information embedded in some abstracts for which the authors have clearly stated distinctive sentence headings.
The authors have had to invent a kind of taxonomy of the local and distinctive, which they have broken down under about 600 headings and then treated alphabetically, each subject receiving on average just under a page apiece.
For example, P headings contain POPULATION, PARTICIPANTS, PATIENTS, SUBJECTS, SAMPLE, etc. Afterwards, abstracts containing distinctive sentence headings were automatically selected and the first sentences following the PICO descriptor marked with corresponding PICO elements.
