During Phase II (post-discharge phone education sessions), what happens if patients cannot be reached?
During Phase II (post-discharge phone education sessions), a total of 11 phone contacts were made with the patient (twice a week for the first 2 weeks, once a week for weeks 3 6, and every other week for weeks 7 12).
