The WHO defines diffusely infiltrative astrocytic tumours with cytological atypia alone as grade II (diffuse astrocytoma), those also showing anaplasia and mitotic activity as grade III (anaplastic astrocytoma), and tumours additionally showing microvascular proliferation and/or necrosis as WHO grade IV.

Astrocytomas can be graded from low-grade circumscribed neoplasm, through diffuse astrocytoma, anaplastic astrocytoma and glioblastoma multiforme, which is a World Health Organisation (WHO) grade IV tumour.
Twelve patients had grade IV glioma (10 glioblastoma, 2 glioblastoma with oligodendroglioma component), 6 had grade III glioma (4 anaplastic astrocytoma, 2 anaplastic oligoastrocytoma), and 2 had grade II diffuse astrocytoma.
