MRI will show a destructive lesion involving the skull base, with low-intermediate signal on T1WI, mixed low, intermediate or high signal on T2WI.

On MRI, they present as a locally destructive lesion with low-intermediate signal on T1WI, intermediate-high signal on T2WI and prominent contrast enhancement.

Interlobular bile ducts were affected by destructive lesions with focal destruction of basal membrane by inflammatory cells, occasionally migrating within the biliary epithelium.

Such lesions are in close topographical relation with inactive destructive lesions (lesion type 2) and we suggest that global overt tissue destruction and the formation of cystic cavities is the direct consequence of complement and granulocyte-induced tissue injury.

However, injury to structures necessary for normal brain development can also occur such that malformative and destructive lesions present simultaneously.

The identification of either further stress fractures or stress-related bone lesions or further lytic destructive lesions usually helps to make the correct diagnosis (Fig. 10).

Osseous involvement in children with LCH is very similar to that seen in adults with multiple myeloma (i.e. predominance of destructive lesions in axial skeleton).
