We coded the communication notations into four categories. 1) Descriptions of communication abilities appeared in 663 (9%) of all encounter notes, 2) descriptions of communication methods appeared in 590 (8%) of all encounter notes, and the last two categories 3) medical management and 4) referrals to speech-language pathology services each appeared in 148 (2%) of all encounter notes.
(Focus Group 2, Nurse 5) The versatility required for patient counselling was underlined by several descriptions of communication barriers with the diverse patient population in this setting, including literacy and mismatch between the care provider and the patient's native language.
We believe that this project will make a significant contribution to research and theory of person-centred communication between care providers and older people by providing descriptions of communication behaviour in practice, by identifying important traits relating to care providers and by examining what factors facilitate or hinder the fulfilment of person-centred communication.
The first descriptions of communication training emerged in the early 1970s [ 7], and the subject is now well established in most medical schools across the US [ 8, 9] and the UK [ 10, 11], as well as some other European countries [ 12, 13].
