Individual performance calculations of two of these gene products, MMP-8 and elastase-2, demonstrate reasonable sensitivity in the derivation cohort (Table 3), but even higher sensitivity in the validation cohort (Table 5).
Patients with a positive microbiological culture (n = 301) had similar baseline characteristics to the entire population in the derivation cohort (Table 1) and had similar allele frequencies to those of HapMap European ancestry data (Table 2).
In each of the administrative validation cohorts, the model fit was similar to the derivation cohort (Table S3).
Unfortunately, baseline hemodynamic, heart failure, and renal function variables were not reported in the original TIMI 11B study to be able to compare associated presenting conditions across the four derivation cohorts (Table S1).
We also compared our results with the HRs of QRISK2 model in derivation cohort (Table 4).
Baseline characteristics for patients in the QResearch validation cohort were similar to corresponding values in the derivation cohort (table 1).
Using the variables and their respective weightings identified in the derivation cohort (table 2), individual risk of AHFS was predicted.
Diagnostic performance of the different cut-offs was comparable to that in the derivation cohort (Table 4).
In the validation set, the performance of the algorithm was in a lower, but similar, range to that in the derivation set (Table 5).
The automated electronic search strategy achieved a sensitivity of 85%9595% CI, 56%-97%) and a specificity of 95%9595% CI, 87%-98%) in the derivation subset (Table 1).
