The rearrangement of the pool of synaptic vesicles from a loose distribution with only a small releasable pool towards a dense organization at the PreAZ is the most important structural modification.
This dense organization of rich club connections was again found in the present study carried out on the regional scale, together with the observation of rich club connections (on average) showing the highest streamline count and spanning longer distances (Fig. 3) as compared to connections linking peripheral, i.e. non-rich club, nodes (van den Heuvel, Kahn et al. 2012; van den Heuvel et al. 2013).
Surprisingly, axonal ultrastructure was well preserved and remarkably consistent between the fixed and fixed-frozen tissue samples collected from each of the cases 1 and 2. Compared to case 1, case 2 though exhibited considerably denser organization of myelinated axons (myelinated axons/μm).
Increased cell size and irregular cell shape might be associated with the dense cytoskeleton organization, which could lead to an increase in both stiffness and viscosity.
A pathological focus within the brainstem affecting both sides symmetrically is plausible, but would likely be accompanied by concomitant clinical symptoms (i.e. motor involvement, cranial nerve abnormalities), given its dense structural organization.
