End-point variables were GP related contacts, out-of-hour services, hospitalization and nursing home placement within a three-year period from enrolment, and a cumulated value of these services.

C OF2 has the lowest absolute cumulative difference with a cumulated value at the end of the simulation period of 3.3 mm above the observations; C OF12 has the largest absolute cumulative difference with a value of 161.2 mm of the objective functions above the observations (Figure 5).
