In Ontario, home care services are provided by the Ministry of Health and Long-Term Care's fourteen regional Community Care Access Centres, who determine eligibility and coordinate providers.
Some patients had numerous attending physicians whose care was poorly coordinated between providers.

Due to technical and legal constraints, it is often difficult to coordinate between providers and thus it is critical to assess the disclosure risk in distributed environments, so that we can develop techniques to mitigate such risks.
That means choosing and coordinating systems providers for things such as financial settlement, fulfillment, delivery and customer-relationship management, to name a few.
Problems typically include difficulties with obtaining needs assessments, putting together comprehensive service packages, coordinating multiple providers and services, ensuring continuity, and monitoring health and functional status [ 14].
The effectiveness of communications can be strengthened by applying knowledge gained from similar efforts in other communities and by coordinating service providers.
Other barriers include a lack of incentives to change practice styles, unclear accountability for health outcomes, competing priorities, lack of resources, health funding constraints, regulation and patient factors (clinical contraindications or history of intolerance to a recommended medication, patient refusal), difficulty coordinating across providers and cultural barriers [ 6, 26– 30].
In terms of recruiting skills, caregivers must know: 1) what outside resources are available and whether these resources are covered by the recipients' insurance benefits; 2) how to screen, secure, and manage paid caregivers; 3) when to involve other informal caregivers; and 4) how to coordinate multiple providers and community resources.
This might be coordinated across healthcare providers rather than being localised to a particular provider to achieve economies of scale.
