Transport costs challenging residents' ability to pay included: public transport from the patient's home to distant health care facilities; public transport from the community level facility to the district hospital, in case of referral; and at times, calling or fueling a government-owned ambulance, in case of referral.
In Bangladesh, which the study notes saw a 60percentt reduction in under-5 deaths from diarrhea from 2005-2015, Save the Children has helped decentralize pneumonia and diarrhea treatment from formal health facilities (mostly in large population centers) to community-level facilities.
However, it does not have contracts with community-level facilities in all areas, which again encourages members to seek care at higher levels.
In 2011, the Heshima project in Kenya launched complementary interventions at community-level, facility-level, and policy-level.
Previous studies indicate structural factors, including lack of financial or economic resources, transportation, and delivery supplies, lack of coordination and referral between TBAs at the community level and facilities can all inhibit women from using facility-based services [ 9, 12- 15].
Government's community level health facilities: Sub-Health Post/Heandhospitalsimary Health Center (SHP/HP/PHC) and hospitals were reported as the primary contact point only in 18.2% and 17.3% respectively.
The place of district and community level health facilities in reducing neonatal morbidity and mortality in developing regions should also be emphasised and committed steps taken to ensure they are appropriately staffed and equipped.
The EmONC package consists of provision of maternal and neonatal health pack (clean delivery kit, emollient, chlorhexidine) for safe motherhood and newborn wellbeing and training of community level and facility based health care providers with emphasis on referral of complicated cases to nearest public health facilities and community mobilization.
Finally, the regressions reported under section 05 consist of varying observations, although very slightly, because of missing or flagged height-for-age data, missing community level health facility information, missing mother's education data or missing consumption expenditure or wealth index information.
In all cases, CHWs are formally trained and engaged with the health system but work at the community level outside of facilities and receive no formal professional or paraprofessional certificate and no tertiary education degree.
Through community meetings, challenges at community level and health facility were discussed and raised with leadership, for example, the lack of housing available for health facility staff (Box 2).
