In Staffordshire, four NHS commissioner groups have got together to tender a 10-year contract for health and social care worth £1.3bn.
Seven states have adopted other consumer protection recommendations approved last year by the state commissioners group.
Kate Dando, a spokeswoman for the commissioners' group, did not respond to questions about the ethics of the arrangement.
The telephone survey of 1,000 people nationwide, conducted May 17-21 by International Communications Research, was paid for by the insurance commissioners' group.
"However, the brief order issued today seem to include a plea of poverty and is inexplicable," said the public utility commissioners group's president, Tony Clark of North Dakota.
They have also provisionally formed a UK commissioners' group, with the aim of establishing common policies on issues such as asylum and the commercial pressures facing children.
In 1989, for example, according to State Insurance Department figures, average premiums increased only 5.1percentt, down from the double-digit increases reported by the commissioners' group in the previous three years.
