While no single definition of the term toolkit has gained wide acceptance, for the purposes of this review 'toolkit' was defined as the packaging of multiple resources that codify explicit knowledge, such as templates, pocket cards, guidelines, algorithms, summaries, and that are geared to knowledge sharing, educate, and/or facilitate behavior change.
