Our findings suggest that an adaptive antiviral strategy with conservative initial treatment levels, followed by a timely increase in the scale of drug-use, can minimize the final size of a pandemic while preventing large outbreaks of resistant infections.
If followed by a timely increase in the level of drug-use, this strategy would preserve the potential for minimizing the final size of a pandemic (Figures 5c,d), while preventing large outbreaks of resistant viruses (Figures 6c,d).
Not only can this adaptive strategy prevent large outbreaks of drug-resistant infections, but it can also minimize the overall pandemic burden if followed by a timely increase in the scale of drug-use.
The results demonstrate that conservative treatment levels during the early stages of the outbreak, followed by a timely increase in the scale of drug-use, can minimize the likelihood of both resistance emergence and run-out.
Previous modelling studies suggest that, in the ideal situation where adequate supply of antiviral drugs is secured, conservative treatment levels at the early stages of the outbreak, followed by a timely increase in the scale of drug-use, would preserve the potential for minimizing the final size of a pandemic while preventing large outbreaks of drug-resistant infections [ 18, 19].
Our results reveal that conservative treatment levels during the early stages of the outbreak, followed by a timely increase in the scale of drug-use, will offer an effective strategy to manage drug resistance in the population and avoid run-out.
