No single blueprint for the service exists: services differ in approach, location, skill-mix of staff and numbers of people supported.
Paul Farmer, chief executive of Mind, who chaired the taskforce, said it was a "feasible and affordable blueprint" for transforming services.
The plans are the vehicles for implementing locally the NHS Five Year Forward View, NHS England chief executive Simon Stevens's blueprint for transforming services to cope with the growing demand for care while also delivering the £22bn of savings he has pledged to find by 2020.
One mechanism by which evidence can be translated into practice and policy is through Models of Care (MoCs), which provide a blueprint for health services planning and delivery.
But he added that NHS England was confident that the major changes outlined in October in its Five Year Forward View blueprint for the service's future, including an unprecedented build up of services delivered outside of hospitals, would help the NHS remain viable.
