We included only the first operations for each patient during the observation period (simultaneous replacement of both hips or knees were, however, included and are referred to as bilateral operations), leading to exclusion of 306 subsequent primary joint replacements.
The inclusion criteria are: traumatic brain injury, age 10 to 65 years, abnormal CT scan and the exclusion criteria: bilateral fixed and dilated pupils, bleeding diathesis, devastating injury not expected to survive 24 hours.
Exclusion criteria included bilateral oophorectomy, active malignancy, previous ovarian cancer and increased risk of familial ovarian cancer.

Exclusion criteria were: bilateral SIS; calcification and fractures (evaluated by X-rays); shoulder instability; rheumatoid, inflammatory, degenerative or neurological diseases; previous neck or shoulder surgery; neck pain, cervicobrachialgia or shoulder pain reproduced during neck movement; shoulder capsulitis.
Exclusion criteria were bilateral TKA, taking warfarin, having contraindications regarding use of any of the study medications, having dementia, or not being able to speak Swedish (due to the high level of language skills required for questionnaires and pain evaluation).
