While it's frequently found that some of the properties in an Action-Form correspond to properties in the model, the ActionForm is used by the JSP to transfer the state between the View and the Model components.
When we select those models that have healthcare specific versions, that are internationally and frequently used and have assumed or proven relations between the models components and better results in health care, only the EFQM quality management model and the Chronic Care Model (CCM) remain.
