In a context where trust between professionals and managers, and between the NHS workforce and government has all but broken down, resorting to more and more regulation is self-defeating.
By identifying how the conceptualization of quality varies across the national, hospital, and micro system levels, and between professionals and managers, the study contributes to a better understanding of the requirements for improved QI processes.
Previous research shows differences in quality conceptualizations between professionals and hospital managers [ 1, 5, 8], within professional groups [ 7, 9, 10], between patients and relatives [ 11], and between patients and doctors [ 12].
Valuable products and services emerge from the clash of ideas between analytical professionals and managers whose greatest strength is their intuitiveness.
But all societies are organized into classes -- those who produce wealth (workers, including the vast numbers in "service industries"), those who control the wealth produced by others (capitalists), in between the professionals and managers who facilitate this process, including university professors, as well as small businesspeople and the self-employed.
The presence of central coordination, competent in scientific methodology and implementation techniques, is necessary to support the work of individual multidisciplinary teams, to favor agreement between health care professionals and managers, and to constantly remind all parties involved of the importance of the program, as motivation is likely to decrease with time.
