The impact of CLSI breakpoint revisions on susceptibilities to aztreonam, cefazolin, cefepime, cefotaxime, ceftazidime, ertapenem, imipenem, and meropenem are also compared using the current breakpoints and those of 2009, the year prior to different revisions [ 25].
However, carbapenem-susceptible P. putida had low levels of susceptibility because the MIC50 was only 1 dilution below the current breakpoint (7, 8 ).
The revised CLSI breakpoints had significant impact on susceptibility to cefazolin (2009 vs. current breakpoints, 71.9% vs. 0.9%) and imipenem (99.8% vs. 55.1%) (P < 0.001 for both).
Resistance mechanisms in these isolates will be undetected if the fluoroquinolones' susceptibility was determined and the results interpreted according to the current breakpoints.
Our results also showed that the revised CLSI breakpoints have significant impact on susceptibility to cefazolin (2009 vs. current breakpoints, 71.9% vs. 0.9%), cefotaxime (95.2% vs. 85.7%), and imipenem (99.8% vs. 51.8%) in P. mirabilis (P < 0.001).
Thus, in the post-PCV-7 era, there were only 4 non-CSF and 2 CSF isolates that were non-susceptible to penicillin using current breakpoints.
However, this is not reflected in current NCCLS breakpoints and hence the fluoroquinolone breakpoints need to be redefined for S. typhi.
Nevertheless using levofloxacin as a marker for this drug class activity, streptococci, several species of Enterobacteriaceae and H. influenzae were highly susceptible using current breakpoint guidelines.
