Patients in the Symbicort® arm are discharged with two inhalations of Symbicort® as maintenance besides additional doses on an as-needed basis for up to eight doses for 30 days.
Four serious issues are identified: patients being discharged before they are clinically ready to leave hospital; patients not being assessed or consulted properly before discharge; relatives and carers not being told that their loved one has been discharged; and patients being discharged with no homecare plan in place, or being kept in hospital due to poor coordination of services.
Over the past two years, at least 91 patients have waited more than a year to be discharged, with at least seven patients waiting more than two years.
The pink colour was discharged with one to two drops of strong acid solution.
It is noteworthy that three-quarters of the patients were discharged with at least three changes to their drug regimens relative to their drug regimens at admission.
Under topical therapy, the IOP was normalized, and upon suspicion of steroid response, the patient was discharged with rimexolone eye drops four times a day, dexpanthenol five times a day and a fixed combination of brimonidine/timolol two times a day.
After 1 week, the patient was discharged with prednisolone eye drops eight times a day (on a tapering dose), dexpanthenol five times a day, a fixed combination brimonidine/timolol two times a day and methylprednisolone 80 mg per OS daily (on tapering dose).
One was discharged with improved condition, while six patients died.
To put this in context, the odds of a patient who is discharged with a score of zero being readmitted within 28 days is 80 1, but for every 7-point increase, the odds double, so that they are 40 1 with a score of seven, 20 1 with a score of 14, and 10 1 with a score of 21.
Of those three patients, one died after developing ALF and the other two were discharged with normal liver function within 72 h.
