Although our initial approach to highly suspicious mediastinal lymph nodes is an EBUS-guided biopsy, false-negatives have been reported with normal-sized lymph nodes or those with minimal FDG avidity, likely due to sampling error, and mediastinoscopy remains the gold standard.
