Salivary cortisol samples were collected to assess HPA-axis function (awakening level, 1-h awakening response, evening level, diurnal slope and post-dexamethasone level).
This interpretation was supported by inspection of visual plots suggesting that the increase in AUCI for CAR-blunted OC participants was due to a lowered cortisol awakening level (sample 1 taken immediately upon awakening), followed by a more dynamic HPA-axis response to awakening (Additional file 5: Panel S2, c-d).

A negative CAR indicated by awakening levels that are higher than awake-plus-30 min levels could suggest that the awakening sample was not provided immediately on awakening (Adam et al. 2006; Chida and Steptoe 2009; Pruessner et al. 1999).
As can be seen in Table 2, mean cortisol concentrations at each time showed a typical diurnal pattern for the at-home and laboratory test day salivary cortisol concentrations involving: (1) higher awakening levels than bedtime levels; (2) peak levels at 30 min after awakening, and (3) a steady decline from the post-awakening peak across the day to a nadir at bedtime.

We confirm several previously published studies, showing no significant differences in awakening cortisol level and/or rise thereof between clinical populations of burnout subjects and controls 13 15 but, as previously mentioned, contrasting results have been reported.
A MANOVA comparing awakening cortisol levels (AUC a ) and diurnal cortisol levels (AUC d ) between subjects in the EO-CD group with elevated CU traits (CU+ group; T value ≥ 70) and subjects of the HC group revealed marginally significant reduced cortisol levels in the CU+ group in multivariate testing (P = 0.076).
In subsequent ANOVAs, awakening cortisol levels were significantly reduced in the CU+ group (P = 0.023; mean AUC a : 496 nmol/L∗h, SD = 151 nmol/L∗h), while diurnal cortisol levels were not significantly reduced in the CU+ group (P = 0.13; mean AUC d = 2376 nmol/h; SD = 1076 nmol/∗h).
