The questionnaire has three subscales: 1. Internal Control, 10 items, average score calculated, range 1 – 5. 2. External Control, 11 items, average score calculated, range 1 5. 3. Support, 12 items, average score calculated, range 1 – 5. Total scores range from 3 to 15.
