The phrase "altered item" is correct and usable in written English.
It can be used to refer to an object or product that has been changed or modified in some way.
Example: "The altered item was no longer eligible for a refund due to the modifications made."
Alternatives: "modified product" or "changed object".
The current CFA analysis was conducted to test fit on the new version of the survey, which included altered item wording and reflected the removal of previous items.
