The phrase "already not able" is not standard in written English and may cause confusion.
It could be used in contexts where you want to express that someone has reached a state of inability at a certain point in time, but it is awkwardly phrased.
Example: "After the accident, he was already not able to walk without assistance."
Alternatives: "no longer able" or "already unable".
