For each item, those rated '4' would appropriately be allocated to high security, those rated '3' to medium security, '2' to a low-secure unit (e.g. psychiatric intensive care or a locked high observation unit), '1' could be safely cared for in an open psychiatric ward and '0' could be followed up as an out-patient.
