In this article, we examine the effects of prior and current levels of AER on the rate of decline in estimated GFR (eGFR) and on the risk of decreased levels of eGFR (<60 ml/min/1.73 m) in subjects with type 1 diabetes in the Diabetes Control and Complications Trial (DCCT) and/or the follow-up Epidemiology of Diabetes Interventions and Complications (EDIC) study.
A unique aspect of the DCCT/EDIC study was the ability to determine the effects of antecedent levels of AER on the risk and rate of progression to eGFR <60 ml/min/1.73 m.
When we assessed the potential benefit of using L-FABP instead of AER for the prediction of progression with ROC curve analyses adjusted for the basic model, we found that the area under the curve (AUC [95% CI]) for L-FABP (AUCL-FABP) was smaller than the AUC for AER (AUCAER) at 0.735 (0.711 0.757) vs. 0.778 (0.756 0.799; P < 0.001), suggesting that AER performs better.
Likewise, FGF8 is downregulated in the AER of embryos carrying the combined loss of Dlx5; Dlx6; two transcription factors causally implicated in SHFM type-I (55).
This result confirms previous in situ hybridization data showing reduced expression of FGF8 in the central wedge of the AER of Dlx5 Dlx6 DKO HLs (19) (our unpublished data).
