The estimate of non-adherence with performance measure does not take into account individual physician practice style or patient preference; the performance measures were designed to ignore such considerations [ 10].
Our study indicates that apparent peer influence on the use of the guidelines makes it feasible to monitor IP-behaviour towards guideline adherence with specific performance indicators, and to provide them with feedback.
Especially, clinician consultations varied markedly in their adherence to performance standards at the assessed clinics.
The challenges include compliance with performance measures, adherence to guidelines, adequate prevention, and early control of comorbidities that affect the progression of AF and associated risks, early initiation of treatment, and successful evaluation of the associated risks of bleeding, primary or recurrent stroke, and patient awareness and compliance [ 1, 8, 13].
In particular, we wanted to assess the risk of SARS-CoV transmission associated with adherence to infection control precautions and with performance of "high-risk" procedures in a setting in which adjustment for potential patient-related characteristics was possible.
Only 1 standard (checking adherence with treatment) showed improvement, while performance declined on 5 other standards and remained constant on 2. The declines exceeded 15 percentage points for 3 standards: ART counseling by pharmacists, individual monitoring plans, and blood drawing.
With regard to adherence measurements, performance for various cut-off points to predict a viral load above 400 or above 1,000 copies/ml is shown in Table 1, Additional file 2, and Additional file 3. The 400 copies/ml virological cut-off was the study's detection limit of viral load measurement, while 1,000 represents virologic cut-off widely used to perform genotyping.
Our findings mirror earlier studies in Cote d 'Ivoire and Uganda which found that most facilities maintained good adherence to standards of quality HIV care only in the first visit with performance declining over time [ 17, 18].
They have been shown to interfere with medication adherence [8,9], workplace performance [10], driving [11] and ability to carry out tasks independently [12,13].
With some exceptions [ 17, 18], this work has mostly focused on staff responses to particular patient safety initiatives conceived in terms of adherence and performance [ 19, 20].
