Add peep-toed wedge heels from the 1930s and a pillbox hat from the 1960s.
The addition of external PEEP does not cause further hyperinflation or adversely affect haemodynamics or gas exchange, provided that the added PEEP is less than approximately 85% of the level of PEEPi [ 20].
As Vd/Vt may be increased by PEEP-induced overdistension [ 26] additional adjustment was performed by adding PEEP into the model.
PEF was measured by Navigator Graphics Monitor (Newport Medical Instruments Inc., Newport Beach, CA, USA) during PEEP zero positive end expiratory pressure (ZEEP), manually assisted cough with baseline PEEP (MAC), mean PEEP of 5.5 ± 1.44 cmH2O, and MAC with adding PEEP of 12 cmH2O.
A few examples: crackles, especially those that blossom late in the inspiratory cycle, suggest the need for added PEEP (either for improving gas exchange or preventing ventilator induced lung injury).
One is to add external PEEP (PEEPe), which reduces the pressure difference between Palv and Pao, and, therefore, the magnitude of Palv reduction required to generate an inspiratory flow.
It has been demonstrated that in patients with chronic obstructive pulmonary disease more than 40% of inspiratory muscle effort can be expended to overcome auto-PEEP [ 41, 42], and that adding extrinsic PEEP can attenuate the inspiratory muscle effort needed to trigger inspiration and improve patient ventilator interaction.
In practice, however, adding extrinsic PEEP in some patients with severe asthma has been shown to worsen auto-PEEP [ 43].
From a pathophysiological point of view, application of PEEP at a level lower than the patient's auto-PEEP (e.g., 80 90 %) is expected to reduce auto-PEEP and leave total PEEP unchanged in the presence of flow limitation, while in the absence of flow limitation the applied PEEP should add to auto-PEEP and increase total PEEP [1, 3 5].
Lengthening the expiratory time, using bronchodilators, adding external PEEP, and reducing the sensitivity of the trigger are alternatives that can improve synchrony.
