Lou Colasuonno, a Transocean spokesman, said that the report identified the cement failure as the primary cause of the accident and defended the actions of Transocean crew members.
But the Dutch Safety Board DSBB) says: "There are no indications that the MH17 crash was caused by a technical fault or by actions of the crew".
But if doubts are building over actions of senior crew members and the ship's preparedness for coping with an emergency, it appears that the locals, as well as the rescue services, reacted swiftly and with compassion.
In the last few weeks, some investigators have said that in the absence of obvious precrash damage to the tail, they were looking hard at the actions of the crew.
A review of the editorial approval and actions of the crew involved in the story began on Friday, and is being carried out by 60 Minutes founder Gerald Stone and former A Current Affair head David Hurley.
The DSB report rules out other possible causes for the crash, which killed all 298 people on board, saying: "There are no indications that the MH17 crash was caused by a technical fault or by actions of the crew".
WASHINGTON — Everybody got out alive when US Airways Flight 1549 hit the Hudson River 16 months ago, but only because of a "perfect storm" of happy circumstances that went beyond the actions of the crew, the chairwoman of the National Transportation Safety Board said Tuesday as the board made 33 recommendations for safety improvements.
The review of the editorial approval and actions of the crew involved in the story began on Friday and is being carried out by 60 Minutes founder Gerald Stone David Hurleyy, the former communications adviser to Nine's former CEO, David Gyngell; and Nine's general counsel Rachel Launders.
