The most effective course of action emerges with time through experimentation and learning during which innovations are adapted locally in an active process of achieving an "intervention-system fit" [ 10].
Although change may be difficult to achieve, an intervention to improve the quality of maternity care could be rewarding, as observed in other studies in Uganda [ 25].
