The phrase "accountable and accountable" is not correct and seems redundant in written English.
It may be intended to emphasize the concept of accountability, but it does not add clarity or meaning.
Example: "In our organization, every team member is accountable and accountable for their actions."
Alternatives: "responsible" or "answerable".
(I should add here that this particular incident notwithstanding, Dallas Police Chief David Brown seems to be one of the better police leaders in the country).
