The phrase "abnormality in the form" is correct and usable in written English.
It can be used in contexts where you are discussing a deviation or irregularity in a specific structure or appearance.
Example: "The doctor noted an abnormality in the form of the patient's heart during the examination."
Alternatives: "irregularity in shape" or "anomaly in structure".
