The phrase "a symptomatic case is" is correct and usable in written English.
It can be used in medical or clinical contexts to describe a situation where a case exhibits symptoms of a particular condition or disease.
Example: "In our study, we found that a symptomatic case is often more challenging to diagnose than an asymptomatic one."
Alternatives: "a symptomatic instance is" or "a symptomatic situation is".
