The phrase "a good self report" is correct and usable in written English.
It can be used when discussing the quality or effectiveness of a self-assessment or personal evaluation.
Example: "In order to improve your performance, it's important to provide a good self report that accurately reflects your achievements and areas for growth."
Alternatives: "an effective self-assessment" or "a strong personal evaluation."
